Access and Functional Needs Program
This program helps identify Chelan County residents who need additional help during an evacuation due to mobility, hearing, speech or other issues that would hamper their ability to evacuate on their own. It is a voluntary program.
Open the forms below in English or Spanish. Download the form, fill it out and save it. To submit the form electronically, click on the "submit electronically" button. You also have the option to mail a hard copy. The mailing address is on the form.
